Adult Outpatient offers individual, group, family therapy and care coordination to assist the client in reaching the goals that they hope to achieve through services. For those who need more intensive mental health care, our live-in facilities can provide 24/7 support for your recovery. Our team will work closely with you and your family to ensure a timely hospital discharge or move-in.

Home health offers patients the ability to be cared for by skilled clinicians in the comfort of their own home, as well as in community board and cares and assisted living facilities. Our Integrated Dual Diagnosis and Intensive Case Management teams provide comprehensive community based services for individuals that are often homeless and in need of support almost daily. At Sequoia Hospice Inc, we use an interdisciplinary group approach of physicians, nurses, social workers, therapists, homemakers, hospice aides, volunteers, spiritual counselors, and bereavement counselors.
